What is it?

The service allows the completion of the introductory medical certificate for:

  • the assessment of the condition of disability, in the experimental phase from 1° January 2025 to 31 December 2026, of the subjects residing/domiciled in nine provinces of the national territory, in accordance with the provisions of Legislative Decree no. 62 of 3 May 2024:
    • Catanzaro;
    • Frosinone;
    • Salerno;
    • Brescia;
    • Florence
    • Forlì-Cesena;
    • Perugia;
    • Sassari;
    • Trieste.
  • the health assessment of civilian invalidityblindnessdeafness, Law no. 104/1992, Law no. 68/1999, for the rest of the national territory, according to the legislation in force until 31 December 2025 (prior to the reform of Legislative Decree no. 62/2024).

Who is it aimed at?

The completion of the introductory medical certificate may be performed by:

  • general practitioners; 
  • primary care paediatricians;
  • outpatient specialists of the National Health System (in Italian, SSN);
  • registered doctors with provisions for pensions;
  • freelance professionals;
  • doctors working at accredited private facilities
  • doctors working at ASL (Local Health Authorities), hospitals, scientific hospitalization and treatment institutes, diagnosis and treatment centres for rare diseases.

How does it work?

By entering the residence/domicile of the claiming individual, the automatic routing service takes the doctor to:

  • the old procedurefor completing the introductory medical certificate, which will also require the submission of the administrative claim to initiate the assessment procedure;
  • the new procedurefor completing the introductory certificate, which directly initiates the procedure for certifying the disability, without further fulfilments required from the individual.
